Appointments

Jose F. Alvarado and Associates takes pride in offering a range of providers and appointment times to accommodate your schedule. Please call our office to schedule any appointments and our front desk staff will accommodate your desired time to the best of their ability. If you are running late or need to reschedule, please contact our office as soon as possible.

Cancellations and No-Shows

We require 24hrs advance notice for appointment cancellation. Last minute cancellations and no-show appointments are subject to a $50 fee charged to your account.

Insurance and Billing

Payment Options

We accept checks, cash or credit cards. Co-payments should be paid at time of visit. If you have question or concerns about payment please ask to speak with our Office Manager, Margarita Alvarado.
